Transcript Request Form 
This form may be completed and returned to the Guidance Department for each college/university to which the student is applying. It should be submitted once the college is added to the list of schools on Naviance. It must contain the college/university name and complete mailing address. Once received, this form gives the Guidance Department permission to release a transcript package via Naviance to that college or university. The student must provide 5 stamps per request form to Guidance for each "mail only" college application.

Graduate Transcript Request
Students that have graduated Mountain Lakes High School should complete this form should they need their transcripts sent to an outside organization. All completed forms should be sent to Mrs. Daniell. 

Document Request Form
Due to the large volume of requests for documents to be sent to outside organizations, students must submit the Document Request Form to the Counseling Department at least one week prior to any deadline. Any request requiring a new letter of recommendation should be submitted at least THREE WEEKS prior to any deadline date.

Release of Records
This "one time" pink form is signed by both student and parent and placed in the Guidance File. It authorizes Guidance to release transcript information to coaches and colleges.

Student Brag Sheet
This form needs to be completed in Naviance.  It provides additional information for the counselor to incorporate into a recommendation letter and is included in transcript packet. (About Me Tab --> Surveys)

Parent Brag Sheet
This form needs to be completed once and turned in to the student's counselor. It provides additional information for the counselor to incorporate into a recommendation letter and is included in transcript packet.

Brag Sheet for Teacher Recommendations
This form needs to be completed and turned in to each teacher writing one of your recommendations. It must be given to each teacher at least 3 weeks before your first college deadline.

Deadlines to Submit Transcript Request Forms
This schedule is a guideline for students to use in providing Guidance with the transcript request forms and other necessary information.

1st Quarter Grade Request
Complete this form if you applied EA or ED to a college and would like your 1st quarter grades sent to any of those colleges. MLHS does not send these grades out automatically since not all colleges require them. That said, students who are proud of their 1st quarter grades are encouraged to send them anyway.

Student Opt-out Form
Under the federal "No Child Left Behind" Act, public high schools must give the names, addresses and telephone numbers of students to military recruiters, college/university recruiters and prospective employers if the recruiters request the information (P.L. 107-110, Section 9528; 10 USC 503). However, students or their parents have the right to instruct the school in writing that this information is not to be released. This is the opt-out document.

Official score reports only need to be sent if a student decides to enroll at that institution.
